THIGH LIPOMAS

Lipomas often arise on the thighs and hips in the original thigh fat tissue. These can be individual lipomas, but not uncommonly, patients have multiple lipomas of the thigh, often in groups, but also individually. The front sides (ventral sides) seem to be particularly affected. Lipomas on the back of the thighs are rather rare. There, they are equally difficult to operate on as back and neck lipomas because the back of the thighs also requires particularly firm and resilient fat tissue for padding and absorbing shear stress on the skin, to withstand constant pressure while sitting, riding, or cycling.
The front sides can be relatively well operated on. However, the "one-extremity-rule" also applies here because you should firmly stand on one leg, while you should at least occasionally relieve the other.

Multiple Lipome

66-year-old patient with numerous, some painful, lipomas on both thighs. Due to the persistent and painful symptoms, she requested the removal of as many lipomas as possible. We adhered to the 1-limb rule, performing the procedure under local (tumescent) anesthesia, and began with the left thigh.

Surgical planning on the morning of the operation for the left thigh. The lipomas were carefully palpated and marked (red lines). Within each lipoma group, "strategic incisions" were planned (green lines) to capture as many lipomas as possible with each incision. The incisions were placed – optimized for aesthetics – precisely along the Skin Tension Lines (STL) to ensure the best possible scar healing.

Multiple Lipome – OP-Planung

The preoperative planning with a "strategic incision" involving only one skin incision. However, as anticipated, this wasn't quite enough, and an additional incision had to be added intraoperatively on the outer (lateral) side to reach as many lipomas as possible. The entire procedure was performed on an outpatient basis under local (tumescent) anesthesia. A total of 16 large lipomas were removed. The operation lasted for 8 hours of "cutting-stitching time" (in addition to preparation and aftercare).

Photo 1: The same patient, 6 weeks postoperatively. It's evident that only two relatively short scars were sufficient to "strategically" remove all 16 lipomas. The overall surface of the body is not affected, the scars will fade and become inconspicuous. Of course, there are still additional (smaller) lipomas present, but 8 hours of surgical time are more than adequate, meaning quite burdensome, and the main troublesome lipomas have been removed. The patient is relieved for the time being. If desired, further lipomas can certainly be removed, according to the patient's personal and professional preferences.

Photo 2: The same patient, 1 year and 9 months postoperatively with healed scars. The scars are not invisible, but not disturbing and have healed in the best possible way from a surgical point of view for the number of lipomas of different sizes that were removed. The patient is therefore completely free of symptoms.
