FOREHEAD LIPOMAS

Lipomas in the forehead area can be quite complicated. They may appear easy to remove, but the opposite is true. Often, patients come to us after unsuccessful attempts or only partial success in removing these lipomas in other surgical facilities. Occasionally, sensitive facial nerves have been injured, leading to numbness in the forehead or front of the scalp. Sometimes, they may recover.
These lipomas almost always lie directly on the bony skull and are scarred with the surrounding tissue. However, I have also (less frequently) operated on forehead lipomas located within the forehead muscle (frontalis muscle).
Previously operated lipomas that were not completely removed are usually embedded in dense scar tissue from the previous surgery. They must be identified in this tissue, and it is not uncommon for sensitive facial nerves of the first trigeminal nerve branch to be involved in the scar tissue and very easily damaged, as they are difficult to identify despite all caution.
The expected scars are usually very inconspicuous or barely noticeable with optimized postoperative care.

60-year-old patient with a "growth" in the middle of the forehead that was causing significant distress, clearly noticeable in all photos, and had already begun to trigger painful episodes ("headaches").

Before lipoma removal
In the forehead area, optimal scarring is crucial. However, postoperative bleeding caused by uncontrollable facial muscle actions can have a very negative impact. Therefore, I suggested a one-time preoperative botulinum toxin treatment as part of the surgical preparation, allowing the tissue to heal postoperatively "undisturbed," somewhat similar to a "cast" for the forehead. These were the injection points for the botulinum toxin. It proved beneficial for the patient, although it wouldn't have been an obligatory requirement for the intended surgery, merely a sensible optimization option.

It's barely noticeable, and there's no tissue indentation. The forehead is smooth. Our patient was satisfied with the outcome.
