Planning of the surgical procedure

This incision deliberately avoids being placed within a forehead fold to prevent deepening the fold through subsequent scar tissue formation. Furthermore, the incision is not placed directly over the growth (which would be the technically easiest approach), but deliberately at the upper (cranial) edge of the growth, outside the glabella (an aesthetically crucial region between the eyebrows and a constant "communicator" in human interaction). It often requires a lot of careful consideration to do "the right thing" surgically.
This is how organized it appears during the surgery,
and the patient is completely pain-free (under local anesthesia). First, the skin with the dermis is addressed, immediately followed by the fine subcutaneous fat layer of the forehead, and beneath that, the thin layer of the expansive forehead muscle (frontalis muscle). Only beneath this lies the round, yellow lipoma, with its lipoma capsule mostly intact, still situated adjacent to the skull bone.
The removed lipoma
The completely removed lipoma is round and placed beside the surgical wound. Below the wound (towards the nose) is a satellite lipoma found nearby, which was later histologically confirmed as such. It's quite evident that the excision leaves a pronounced indentation on the forehead (like a valley). A simple closure would leave an aesthetically significant defect. Hence, I meticulously mobilized the tissue beneath the skin (primarily as a muscle flap graft) and reconstructed the central forehead muscle. This approach was very rewarding in the end.

The lipoma capsule is intact. Please note: Every lipoma shrinks by at least one-third of its original size after being removed from its fibrous attachment within the body. Always.
