FOREARM LIPOMAS

On the forearm, there is essentially only the natural layer of subcutaneous fat tissue beneath the skin from which lipomas usually originate. Between this fat layer and the forearm fascia (a flat, tendinous sheath covering the arm muscles), there is a thin plane with fine, longitudinally running sensitive nerve bundles responsible for the sensitivity of the forearm and hand, fine arterial blood vessels, larger veins (from which blood is usually drawn by doctors), and delicate lymph vessels. Often, lipomas of the forearm are fibrously connected to underlying nerves (tendon-like), or lymph vessels may run over the surface of the tumor's capsule. Larger veins and arterial vessels can also be "integrated" with the lipoma capsule. It is better to microsurgically separate them from each other to preserve them optimally. Scars are important on the forearm because they are in an area frequently not covered by clothing (short-sleeved shirts or blouses). Therefore, especially here, an optimized postoperative treatment is recommended to achieve as inconspicuous scars as possible.

This very likable 38-year-old patient suffers from numerous lipomas inherited through his family affecting his entire body. His left forearm was his primary concern, and it was the first body region operated on following the "one limb rule."
Since he is an educated individual who primarily works at a desk professionally, the lipomas on the ulnar aspect, which is the part of the forearm that rests on the desk's edge, were particularly bothersome.

The dotted lines represent the palpable boundaries of the lipomas. The numbers denote each individual lipoma to be removed so that there can be an exact correlation between the examined tumor from the pathologist and its location on the body. This can be especially important if, in a very rare case, a lipoma degenerates into a liposarcoma, is recognized as such by the pathologist, and targeted reoperation at that site becomes necessary. The solid lines indicate the intended length and direction of the incisions along the skin tension lines (STL) for optimal aesthetic scarring results. The incisions were strategically placed so that at least lipomas 1 and 2, as well as 3 and 4, could be removed with minimal scarring from each incision.
