FACIAL LIPOMAS
The face contains many sensitive and motor nerves. Their injury would lead to either localized loss of skin sensitivity or partial facial muscle paralysis ("crooked face"). Therefore, such tumors must be operated on with the utmost surgical caution. I do not see this as a major problem if I am allowed to take the necessary operating time for the required precision. I am very familiar with the anatomy of the face (I am "at home" here), and there is also a very important trick: if I create a clean, bloodless, intraoperative anatomical overview under optimal bipolar hemostasis (the time factor!), I have the opportunity to move microsurgically and accurately on the surface of the lipoma capsule because the capsule does not contain any nerves, so I am "safe". The problem here is usually that the surrounding tissue tends to fall back into the surgical field due to the displacement caused by the lipoma growth. Therefore, almost always, qualified medical assistance is needed (the cost factor!) to hold the tissue aside safely and carefully so that surgery can be performed in the depth of the tissue with a good view on the surface of the lipoma capsule. So far, I have had no nerve injuries in the face.

A 71-year-old man with a very large lipoma on the glabella (the region between the eyebrows) and forehead, suspected to extend into the eye sockets. The tumor was completely painless. The patient came to us only after strong insistence from his wife.
The direct preoperative comparison between the "native" condition and magnetic resonance imaging (MRI). The success of the surgery depends on meticulous preoperative planning.
The obligatory MRI (magnetic resonance imaging) examination of the head in this case was aimed to determine the depth of tumor invasion and whether the eye sockets were affected. Fortunately, this was not the case, allowing us to proceed with an outpatient surgery.

The scar is barely noticeable, and the forehead can be wrinkled, indicating that the nerve supply to the frontalis forehead muscle is not compromised.
2 weeks postoperatively, the glabella muscle of the forehead can be optimally activated again. This indicates that all facial nerves were ideally preserved during the surgery.
