AXILLARY LIPOMAS

Lipomas in the armpit can be quite tricky. On the one hand, all the nerves and blood vessels of the arm pass through the armpit; on the other hand, the way into the chest (thorax) with the lungs and into the mediastinum (the space where the heart with the largest blood vessels of the human body is embedded) is very short and surprisingly easy. Furthermore, many lymph vessels and nodes are located here, and their injury can easily lead to chronic swelling of the affected arm. Surgeons who underestimate these factors can quickly find themselves in critical surgical situations. Therefore, everything already depends on the correct surgical preparation. Not to mention the obvious, extremely cautious surgical approach.

A 52-year-old woman with an axillary (armpit) lipoma nearly the size of a handball on her right side, which was almost as large as one of her breasts. Her primary care doctor had advised against surgical removal for years, until the painful lipoma had grown to a bothersome size. We performed the extensive procedure to remove this lipoma under general anesthesia in a hospital setting.

The scar healed without complications 3 weeks postoperatively. The large defect left by the removal of the lipoma was filled with high-quality local healthy tissue, using fat flap techniques. The residual indentation seen is due to a dissolvable suture and was completely resolved over time.

14.5 years later
the scar is barely recognisable and the armpit has a completely normal contour. The patient is entirely symptom-free. The right arm is fully usable and shows no tendency to swell. The skin retraction from back then has completely disappeared.
