NECK LIPOMAS AT THE BACK – SURGERY PHOTOS
Photo 1: The dissection of the very large lipoma is challenging because the tissue in the neck and upper back is very scarred (fibrous), and great care must be taken not to damage the many nerves that emerge from the cervical spine. The lipoma on the right side of the patient has already been largely dissected and is being held aside with a fine hook.
Photo 2: The lipoma needs to be extensively detached from the underside of the skin, moving upward toward the head. The scissors in the wound indicate the depth of the dissection cavity.
Photo 3: The same situation as in Photo 2, except that the scissor penetration depth is shown above the skin.
Photo 4: After the complete removal of the very large lipoma, a deep crater-like soft tissue defect is left due to the compression of healthy soft tissues by the growing tumor.
Photo 5: A caudally (toward the feet) vascularized fat flap has already been prepared.
Photo 6: The fat flap has been introduced into the soft tissue cavity and fixed in place with absorbable sutures. The tissue defect has been filled with local healthy fat tissue.
Photo 1: The removed, large lipoma with extensive lobulated features and the attached connective tissue of the lipoma capsule.
Photo 2: The lipoma displays large, cone-shaped protrusions that likely compressed nerve pathways in specific areas, ultimately leading to progressive arm paralysis.
