CAN LIPOMAS ACTUALLY BE SUCTIONED (LIPOSUCTIONED)?

Lipomas consist of slightly (benign) degenerated fatty tissue. So, shouldn't they be able to be "easily and with minimal scarring" removed? Superficially, this may seem plausible. Especially considering the goal of preserving the attractiveness of an intact skin surface, this would understandably be desirable. However, with such thinking, both the surgeon and the patient overlook two mothers of medicine: Mother Anatomy and Mother Pathology.
It may not be difficult to insert a suction cannula into a larger lipoma. However, as a surgeon, I have to "blindly" violate the integrity of the lipoma capsule to even access its contents. This creates a tissue channel in which pathological lipoma cells are inevitably transported due to the often repeated back-and-forth movement of the cannula and can settle there, similar to the regular use of healthy fat cells in autologous fat tissue transplantation (AFT).
Furthermore, in this procedure, one would want to be as thorough as possible and remove many fat cells. This inevitably results in the perforation of the lipoma capsule at numerous points by the tip of the cannula, further spreading the pathological fat cells in the immediate vicinity.
Last but not least, it is not possible to completely suction out the lipoma capsule (with the attached lipoma cells). A reliable, complete removal of the lipoma is therefore not possible. Recurrence is inevitable, leading to subsequent surgeries.


Here's a patient story related to this:

I recently performed a follow-up surgery on such a patient. She had undergone a "lipoma suction" on her upper arm by a doctor internationally known for his liposuction procedures because she "feared ugly scars." After several years, a lipoma larger than a man's fist had developed on her upper arm, which proved to be more complicated than expected during our surgical removal because the entire tissue surrounding the lipoma was properly scarred. The postoperative skin scar became inconspicuous with appropriate care, ultimately being less noticeable than the former suction scars.
This case demonstrates that careful surgical procedures and postoperative treatment need not cause unnecessary fears of scars.
On the other hand: Very, very rarely (to my knowledge, 1:10,000 to 1:20,000), lipomas can degenerate into so-called liposarcomas (dangerous cancer). If such a "lipoma" were to be "suctioned" without knowledge of the histological diagnosis, the patient would have almost no chance, as the tumor cells would likely have already spread through the blood and lymphatic system due to such an aggressive procedure, resulting in metastasis. A prudent, gentle, and complete surgical removal with all the necessary measures still offers a realistic chance for the affected individuals, especially for not too large sarcomas.
